Games on Friday nights and Saturdays
Practices on Monday, Tuesday, or Thursday evenings
$45 fee covers the team t-shirt, officials, and facility use
Four co-ed leagues & Two Men’s leagues
5-7 yr olds (recommended for kindergarten to second grade students)
8-10 yr olds (recommended for third through fifth grade students)
11-13 yr olds (recommended for middle school students)
14-17 yr olds (recommended for high school students)
18-30 (Men’s League)
Over 30 (Three-Man Half Court League)
Register by mailing in this registration form along with your payment
payable to: Pine Forest UMC
or drop them off at the church office: Monday-Thursday, 8:30-4:30PM, or go online to register at: www.pineforestumc.org
REGISTRATION DEADLINE IS SATURDAY, FEBRUARY 13TH
Payment must accompany registration form!
Coaches, Assistants & Volunteers are needed for all leagues.
